You must take care in answering all the following questions which are relevant to us in providing this insurance and setting the terms and premium. Please contact us if you do not understand the question or the nature of the information required or please seek guidance from your broker. Failure to provide information or the provision of incomplete or inaccurate information may result in the loss of cover or other remedies.

You must in completing this proposal form make full and proper enquiry of all relevant parties in relation to the answers given and all information and/or documentation supplied with or in relation to any question raised in this proposal form.

You must tell us as soon as practicably possible about any changes to the information you have provided to us which happens before or during any period of insurance. We will tell you if such change affects your insurance and if so, whether the change will result in revised terms and/or premium being applied to your policy. If you do not inform us about a change it may affect any claim you make or could result in your insurance being invalid

Presentation

This proposal form must be completed by an authorised individual; your partner, principal, director or member.

All questions must be answered. If there is insufficient space to provide answers, additional information should be provided on your letter-headed paper.
Where available, brochures, standard contract conditions, agreements and letters of appointment should be provided. if there are no standard contract conditions, supply copies of non-standard contracts.

Where details of your web address have been provided any information contained within or linked to it will be treated as not having been provided unless such information is specifically supplied to us in written form.

Data Protection Act

Any information you have provided will be dealt with by us in compliance with the provisions of the Data Protection Act 1998. For the purpose of providing this insurance and handling or any claims or complaints which may arise under it, we may need to transfer certain information which you have provided to other parties. By signing this proposal form you agree that such transfer(s) may be made.

Turnover is the total income received by a business in a certain period. It's sometimes referred to as ‘gross revenue’ or ‘income’. It is not your business’s profit, which is a measure of earnings after all costs have been paid such as tax, wages and other expenses.

Policies and Qualifications

  • written policies are in force for all of the following:
    • Health & Safety
    • Equality
    • Protection of children and vulnerable adults (including CRB and DBS checks)
  • all coaches / referees / instructors must be registered with and qualified to the minimum standards of the relevant governing body
  • all regulations for the maintenance and safety of your equipment must be complied with
  • all premises used during the course of your business must be maintained to a good standard
  • hazardous substances are not used and/or stored at the premises

About the building

Vacant or Unoccupied Premises

  • all electricity, gas and water main supplies (save as required to maintain protection and security systems) disconnected
  • all water and heating systems drained
  • all outside doors locked
  • all letterboxes and other openings sealed
  • all windows closed and secured
  • if the building has not been in normal occupation for more than six (6) months, are all ground floor windows and (save for one access door) all doors bricked or boarded up
  • all waste, combustible materials and gas bottles been removed
  • to carry out weekly internal visits, remedy any defects found and keep a record of all such visits and remedial work

Cover is provided on the following basis:

Security Requirements

  • all final exit timber doors to must be fitted with frames at least 45mm in thickness and mortice deadlocks confirming to British Standard 3621:1980.
  • all double leaf final exit timber doors must also be fitted with security bolts at the top and bottom of their first closing leaf.
  • all final exit aluminium doors must be fitted with swing bolt mortice locks and all final exit double leaf aluminium doors must be fitted with security bolts at the top and bottom of their first closing leaf.

Fire and Flood Risks

  • the premises must have suitable fire extinguishers and/or fire hoses
  • all waste and refuse is must be removed from all buildings or structures daily and removed from the premises at least once every week.
  • all combustible waste, refuse and stock which is in the open (i.e. which is not kept inside a building) must be kept at least six (6) clear meters away from any building or other structure.
  • highly flammable liquids (any liquid with a flash point below 32 degrees centigrade including and not limited to cellulose nitrate) must not be stored on the premises
  • any heaters used on the premises must be kept at least two (2) clear meters away from any combustible items.
  • all pipe work and tanks at the premises must be appropriately lagged and all stock and materials, including perishable items must be stored at least 15cm above floor level.

Deep Fat Frying Ranges

  • all equipment must be fitted with a thermostat which prevents the temperature of cooking oils or fats rising above 205 degrees centigrade, and a cut-out device which automatically cuts out the heat source if the thermostat fails
  • all equipment (and any extraction hoods, canopies, flues and ducting) must be securely fixed and free from contact with combustible material
  • all equipment must be wiped down at the end of each day
  • all filters and grease traps cleaned inside and out at least once every month, with a written record kept
  • all extraction hoods, canopies, flues and ducting cleaned inside and out at least once every twelve (12) months by a specialist contractor, with a written record of the contractor’s work kept
  • all greasy waste and cloths kept in metal receptacles with metal lids during the day, taken outside the building at the end of each day, and removed from the premises at least once every week
  • all cooking fume extraction systems and any extraction hoods, canopies, flues and ducting cleaned inside and out at least once every twelve (12) months by a specialist contractor, with a written record of the contractor’s work kept

  • the premises nor any tenant of the premises have never been the subject of a complaint, notice or caution

  • civil or criminal proceedings have never been brought against the holder of the licence

Business Interruption

Only complete this section of the form if you have indicated that Business Interruption cover is required. Please note that cover for auditors’ charges is included within the sums insured for all of the Business Interruption covers below.

Employers Liability

Do you need to supply an Employer Reference Number?
Unless all of your employees are paid below the PAYE threshold, you will have been supplied an Employer Reference Number (ERN) by HMRC. Businesses are required to provide their ERN when taking out Employers’ Liability insurance.
Important
You must contact us with your ERN as soon as HMRC have issued it. If we do not hear from you within 8 weeks of purchase we may cancel your Employer's Liability Insurance

  • a written policy is in place with regard to the protection of children and vulnerable adults (including CRB and DBS checks)
  • procedures are in place to control all incoming and outgoing money?
  • financial statements are prepared by a qualified accountant.

Please note that under the policy a ‘Trustee’ is any duly appointed trustee or committee member.

The information provided in connection with this proposal form is true and I/We have not withheld any material facts.

I/We understand that non-disclosure or misrepresentation of material facts will deem the Contract of Insurance void.

I/We understand that the signing of this proposal form does not bind me/us to complete the Contract of Insurance. However, I/We agree that should a Contract of Insurance be completed, then this Proposal Form and the Declaration shall form an integral part of the basis of the Contract of Insurance.

It is understood by the assured that any information provided to the Underwriters regarding the Assured will be processed by the underwriters, in compliance with the provisions of the Data Protection Act 1998, for the purpose of providing insurance and handling claims, if any, which may necessitate providing such information to third parties.

I/We understand that any change in information must be notified immediately and no cover exists until such change has been approved by Underwriters.
